Responsibilities
- Provide skilled nursing care to patients in their homes following established care plans
- Assess patient health status and communicate changes to physicians and care team members
- Administer medications and treatments as prescribed
- Educate patients and their families on disease management and health maintenance
- Maintain accurate and detailed patient records and documentation
- Collaborate with interdisciplinary team members to ensure coordinated patient care
Requirements
- Valid Registered Nurse (RN) licensure in the state of Michigan
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Reliable transportation to travel to patient homes in Saline, MI
